If this new employee is going to be using an already deployed computer, please enter the CPR asset tag in the text box at the bottom of the section.

    All CPR staff are automatically given access to the workgroup@ and announcement@ email groups.

    ***To ensure accuracy, please list the exact name of the email accounts needed, as there are several very similar accounts***

    Please provide the following information regarding the new hire's access to different sections of the Aura EMR. Please select at EACH option that is applicable.

    Please indicate which additional accounts will be needed for the new hire. Any accounts not listed below are not managed by CPR's I.T. department.
