  • We occasionally encounter employees who find it challenging to navigate certain periods of financial difficulty and uncertainty as the result of an unexpected hardship. When these circumstances arise, we will consider a salary loan for employees who qualify. This is an informal program of support and is not intended to be published or advertised as an additional benefit.

  • Salary Loan Request Process:

    1. Completion of the "Salary Loan Request" Jotform and Attestation
    2. Executive consideration and/or approval
    3. HR notifies of approval (or rejection) to employee and Payroll
    4. Payroll will issue funds and explain the repayment schedule

  • Attestation: 

    • I understand that a salary loan is not an obligation of the company to provide and that my request can be denied with or without a reason.
    • I understand that if I have a current loan it must be repaid before a new loan will be considered. Further, I will not be considered for future loans until two years after the date the current loan has been repaid.
    • I agree that I am requesting these funds in good faith and I have taken reasonable steps to manage my financial affairs before this request was made.
    • I certify that I have been employed with CPR for at least 90 days.
    • I understand that my request will not be considered if I have given either notice of resignation or have expressed intentions to resign.
    • I agree that I have no documented performance or attendance issues in my personnel file. 
    • If approved, I understand that the loan amount will be the lesser of my gross paycheck or $2,500.00.
    • If approved, I agree to repay the loan within 6 months of receipt. 
    • I agree that if my employment is terminated (voluntarily or involuntarily) and I have an unpaid loan balance, any remaining amount due will be deducted from my last paycheck. If my last paycheck does not cover the full unpaid loan balance, I agree to make arrangements to repay the loan within 60 days of my last day of employment.
